We are looking to a Team Leader for our START program!

 

Basic Function:

 

To provide supervision, management and leadership to NH START coordinators.  Working in collaboration and following IOD standards, the START Team Leader will provide clinical and systemic leadership to the Collaborative as a whole maintaining communication with other statewide START program collaborative members. The START Team Leader is responsible to communicate on an ongoing basis any changes and services provided by the START Program and keep the agency updated on initiatives and needs of the collaborative.  Communication and ongoing supervision of the START team Leader role resides with the NH Statewide START Director.

 

Minimum Qualifications:

Education:  Minimum of a Master's degree in mental health counseling, social work or other human service related field with at least two years of experience in the intellectual and development disability (I/DD) and/or mental health field. 

 

Experience:  Previous leadership experience with five years of supervisory experience preferred. Extensive experience in I/DD, specifically with individuals that have challenging behaviors is preferred.  Family support and systems knowledge is also preferred. Certified START Coordinator s are preferred to progress to this role. 

 

 

Specific Duties and Responsibilities:

  1. Ability to perform all duties of the START Coordinator (may hold a caseload of up to five individuals)
  2. Supervise/oversee NH START Coordinator Collaborative, crisis team will attend all management training

session provided by Community Bridges.

  1. Represent the agency, the values and mission of the agency, and person centered thinking in all interactions
  2. Effectively communicate with NH Statewide START Director on evolving role of the Team Leader
  3. Provide effective supervision for staff that report directly to you providing feedback, training, performance reviews, development plans, and overseeing time and productivity measures
  4. Establish community linkages and serve as liaison to mental health providers and other community partners
  5. Work in close collaboration with the CENTER for START Services and the Bureau of Developmental Disabilities to identify systems, trainings, service and other needs and insure fidelity to the START model
  6. Provide clinical and systemic leadership to the Collaborative as a whole
  7. Assist in the coordination of CET presentations
  8. Oversee the maintenance of a list of individuals being served along with support being provided
  9. Assist with the preliminary assessment (formerly known as intakes) and case assignment process
  10. Identify and coordinate necessary trainings for team members with the Center for START Services
  11. Maintain communication with other statewide NH START team leaders and participate in national team forums and study groups, as recommended by the Statewide Director or the IOD
  12. Ensure the collection and reporting of required data and documentation on consumer access and utilization of START Services
  13. Meet regularly with designated administrator for each area agency in the collaborative
  14. Provide support to individual START Coordinators on an ongoing basis in cooperation with the Center for START Services
  15. Facilitate the triage and support for emergencies as they arise
  16. Assist in the development of policies and procedures with the Center and other teams
  17. Provide crisis support as needed to the START Coordinators
  18. Serve as a primary liaison to New Hampshire Hospital for the START team
  19. Oversee business hours on-call system for the collaborative
  20. Assist with the development and implementation of additional START services including but not limited to: crisis and planned respite, mobile crisis prevention, and intervention services, and the establishment of improved mental health service capacity
  21. Participate in all statewide forums for NH-START
  22. Coordination of CET's (distribute and collect attendance sheets, CEU sheets, surveys; coordinate videoconference and presentation schedule; coordinate snacks)
