Basic Function:

Participates in transdisciplinary assessments and the writing of assessment reports, providing expertise the areas of motor development assistive and adaptive equipment; participates in the development of the IFSP, provides individual treatment; service coordination, offers parent education and support.  Also provides consultation to other staff and other agencies with whom we collaborate. Must have knowledge and comfort with providing home based services within a transdisciplinary model.

 

Qualifications:

Education:  B.S. or M.S. in Physical Therapy.  Must have a valid New Hampshire state license in Physical Therapy or documented ability to obtain license within thirty days of hire. 

Experience:  Pediatric 0-3 required; home based experience and experience participating on transdisciplinary team preferred.

 

Specific Duties and Responsibilities:

  • Serves families regardless of; disability race, religion, sex, ethnicity, illness, sociological or financial circumstances.
  • Works flexible hours to meet families needs through out service area.
  • Provides physical therapy in home/community environments for eligible children.
  • Participates in transdisiplinary assessments.
  • Writes multiple areas of assessment report.
  • Has strong knowledge base in typical and atypical infant and toddler development.
  • Provides transdisciplinary services and physical therapy treatments.
  • Attends and participates in group and individual supervision, administrative meetings, in services and other training events.
  • Assists family members with management issues and coordination with other community agencies, including writing letters of medical necessity.
  • Works closely with Family Support in meeting the families' needs. Provides       guidance, support, and information to families and helps to coordinate with community agencies.
  •  Maintains records and documents services as required.
  • Maintains daily schedule according to assigned caseload.
