Community Bridges is an area agency celebrating over 35 years working with individuals who experience developmental and intellectual disabilities. These individuals and their families work with Community Bridges to manage their unique challenges, maneuver the obstacles and thrive in their communities.

 

Basic Function: 

Participates in transdisciplinary assessments and the writing of assessment reports, providing expertise the areas of family systems, psycho-social issues, adult and child psychology and behavior and social/emotional development, participates in the development of the IFSP, provides transdisciplinary services, family therapy; service coordination, offers parent education and support.  Also provides consultation to other staff and other agencies with whom we collaborate.

 

Qualifications:

Education: Master's Degree in Counseling Psychology, Social Work, Family Therapy or related field;  license preferred.

Experience: Minimum of two years' experience in the mental health field required; home based experience and experience participating on transdisciplinary team preferred. Must have a valid NH driver's license, a safe driving record and has access to a reliable vehicle.

 

Specific Duties and Responsibilities:

  • Serves families regardless of; disability race, religion, sex, ethnicity, illness, sociological or financial circumstances.
  • Provides services in home/community environments for eligible children.
  • Participates in transdisiplinary assessments and writes multiple areas of assessment reports.
  • Complies with State and Agency policies regarding confidentiality.
  • Provides transdisciplinary services, therapeutic and behavioral support.
  • Attends and participates in group and individual supervision, administrative meetings, in services and other training events.
  • Assists family members with management issues and coordination with other community agencies.
  • Provides guidance, support, and information to families and helps to coordinate with community agencies.
  • Maintains records and documents services as required.
  • Maintains daily schedule according to assigned caseload.
  • Provides parents with support, information, assistance and advocacy throughout services and transition process.
  • Keeps abreast of new developments in the field and attends relevant continuing education events.
