We are looking for an EFC Behavioral Program Manager!!

Basic Function:

Responsible for all aspects of service provision in a variety of settings. Will oversee the  DHHS certification process and assure programs are meeting the regulation standards. Will ensure that team adheres to agency policies and procedures and provide guidance on policy and procedures.  Responsible to uphold the delivery of  quality services.

 

Qualifications:

Education:               Bachelors Degree (Associates Degree plus 3 additional years of experience)

Experience:             At least two years experience working with people who have a disability, with a minimum of one year of supervisory and training experience.

 

Specific Duties and Responsibilities:

  1. Follow Functional Behavioral Assessment (FBA) recommendation and the implementation of appropriate behavioral strategies/plans, utilizing current best practice for Positive Behavioral Supports and in compliance with Regional Behavioral Standards.
  2. Provide supports to staff and Home Providers related to: Positive Behavioral Supports, including behavior as  communication, supporting varied learning styles, and instructional methods based on current best practices, and other topics as requested/appropriate.
  3.  Provide positive interactions to assigned individuals as outlined in the approved support plans. Work on and provide updated support plans, with the ability to edit, recommend, and develop individuals support plans as needed.
  4. Provide support to home providers to help understand individuals support plans and develop strategy to support individuals with on-going challenges. Working collaboratively with all team members for person centered approach to plans and needs of individual.
  5. Responsible for coordinating with clinical consultants to help develop individual's behavioral plans as needed and add recommendations and supports to plan.  Responsible for sharing information and training to staff and support personnel.
  6.  Responsible for behavioral tracking tools,  and interpretation.
  7. Responsible for tacking individual's progress with individual's behavioral plan, updating the plans and/or consulting with the clinical person to update plan in timely manner, and meeting compliance deadlines for such.
  8. Responsible for collection of relevant supporting documentation required for developing or renewing individuals behavior plans and submitting these to case management  by the appropriate deadline to be reviewed by HRC/IRC committees.
  9. Responsible for updating recommendation(s) received from committees and submitting required documentation as stipulated, obtaining approval, record keeping, and adhering to deadlines.
  10. Responsible for implementing approved behavioral plan including training of the support staff and home providers and recording the training for quality measures
  11. Provide consultation and support to Program Managers, coordinators, and staff from other units when there is need for behavioral support.
  12. Attend seminars and trainings to maintain knowledge of best practices in the provision of Positive Behavioral Supports to individual.
  13. Encourage and assist individuals in accessing and fostering community supports and resources such as social/cultural groups, stores, restaurants, employment opportunities.
  14. Perform all duties in a manner that demonstrates an awareness of and respect for each person's legal, civil, and human rights'including maintaining confidentiality of information regarding the individual and that ensures the respect and dignity of all persons who receive supportive services.
  15. Provide all supports in a manner that takes into consideration the preferences of individuals and which actively encourages choice making by the individual.
  16. Act as a positive role model as a means to educate the community and to eliminate preconceived notions and stereotypes about persons with disabilities.
  17. Supervise and monitor all support situations as assigned, such support to include regular home visits. Ensure documentation and follow up accordingly.
  18. Develop, implement and monitor program budgets as assigned.
  19. Analyze and compile relevant data and information for each individual served to ensure that each person's services and supports are effective agency, as well as team building and other topics geared toward enhancing the lives of people served and improving the skills of staff. Provide routine and regular supervision of employees and contracted providers, in accordance with agency policy/standard.
  20. Represent the agency in a professional manner at all times, including adherence to agency policies and procedures.
  21. Attend the service plan meeting, team meetings, agency and staff meetings and other meeting and/or conferences as requested by supervisor.
  22. Ensure compliance with applicable state regulations and agency policies and procedures including: state certification of homes, life safety inspections, employee time sheets, behavioral plan data and reports, medication logs, attendance sheets, fire drills, and other pertinent documentation as requested.
  23. Provide responsive and clinically/medically sound services and supports in accordance with individual needs, desires and dreams.  Such supports will promote the dignity, respect of all individuals
  24. Work cooperatively with team members, families and guardians, and external entities.
  25. Promote and maintain satisfactory public relations between the agency and the community.
  26. On call responsibilities in response to department needs on a rotating basis.
  27. Perform additional duties and responsibilities as assigned
  28. Distribute medication in accordance with He-M 1201 as applicable.

 

Required Skills:

  1. Must be able to provide thorough functional behavioral assessments, develop and revise on an ongoing basis
  2. Share Behavior plans based on current best practices, and associated task
  3. Able to communicate thoughts and ideas well in both verbal and written formats in either group or one to one settings as required.
  4. Ability to access community resources, activities and social gather
  5. Previous experience in  advocating for, providing support to, and assisting individuals to advocate for their right
  6.  Must be able to represent Community Bridges services to the public and maintain a positive image to the community at large
  7. Skilled at working as a member of a team and carry out team decision
  8. Ability to collaborate with individual families, team members and community citizens on behalf of each individual.
  9. Ability to successfully complete and maintain Medication Administration training.
  10. Ability to successfully complete and maintain MOAB mandatory training and certification
  11. Ability to assist in the household maintenance of individuals' home
  12. Ability to assist individuals with bathroom needs and attend to personal hygiene n
  13. Ability to successfully complete and maintain First Aid and CPR training.
  14. Ability to frequently travel (access to personal vehicle during all scheduled work hours) with any individual supported utilizing personal vehicle as well as Community Bridges' vehicle must possess and maintain a valid NH license and safe driver's record in accordance with Community Bridges standards with adequate liability insurance.
  15. Ability to acquire knowledge of Community Bridges mission, values, policy and procedures
  16. Ability to acquire computer competent
  17. Ability to work a flexible work schedule
