We are looking for a Certification Coordinator!!

The Basic Function:

The Certification Program Coordinator is responsible to provide certification related administrative duties 80% of the time preparing for certification, and Quality assurance of certified programs and care tasks 20% of the time. Responsible for all certification related activities in community residences and tasks as needed by the Residential and Day Services Program. Assists in QA/administrative functions for nursing team.  

Minimum Qualifications:    

Education:  Associates Degree preferred.   

Experience:  2 years' experience in the human service or related field with persons with disabilities desirable.  Demonstrable technology competencies required. Administrative experience working in a fast paced environment is necessary. A high level of attention to detail, organization of paperwork, and ability to work on multiple projects required.  Professional communication skills with ability to train others in proper documentation is needed.

Specific Duties and Responsibilities:

 

  1. Collect information about new or existing individuals understand the support needs (read reports, ISP)
  2. Complete initial visit:  visit EFC home in preparation of placement.
  3. Check new homes to verify certification compliance (windows, doors, smoke detector, safety, other concerns) Hard wired/ interconnected Smoke detectors every bedroom, Hard wired/ interconnected Smoke detectors and Carbon monoxide every floor, Hard wired/ interconnected Smoke detector attached garage/attic inspect Individual room and ensure it is acceptable space, closet, address any safety concerns, ensure there  two means of egress windows 5.7 sq feet, GFCI installed - 3 feet of water source (bathroom/kitchen).
  4. Life Safety Report- Contact or coordinate with Fire Department, pay/reimburse fees /dues, attend appointment, and obtain approval or concerns needing corrections.
  5. Complete request home modification- collect quote and process with case manager
  6. Back ground check -Process documentation for provider and family background and review the result for any violations
  7. Waiver- review complete waiver application process for any violation. File waiver information and update in quarterly certification data sheet. Process before 90 days before expiry.
  8. Collect individual information for certification application (Review ISP, Day Support hours, hours of supervision, funding type, date of birth, gender)
  9. Complete Certification Application-start date, number of bed, name telephone of  home provider, manager and case manager information, driving direction, day program supporting information,
  10. Complete Emergency application- individual name, DOB, effective date (can back date 7 days), reason, floor plan with -bedroom with individual initials, smoke detectors marked. Email application to the State
  11. Certification renewal Submit new program application for certification. Track regular certifications and email managers 90 days before cert expires, prepare and submit renewal application. Submit emergency cert application in consultation with program manager and tracked and submit for renewed for additional 45 days.
  12. Provider Training: Insure required home provider trainings are completed and records available in file
  13. Residential Book and Documentation: Set up books for new residences and update existing books, visit program location to insure that residential books are updated regularly. Proactively prepare books and documentation for certifications.
  14. Schedule a regular home and staff residence visit for QA.   
  • Bedroom not common area (connecting exit/rooms/storage/ utilities)
  • Adequate heating and cooling
  • Furnace - cleaning records (Annual) or manufacturers recommendation documentation.
  • Wood burning fireplace (Cleaning by professional and record)
  • No open electrical connection
  • Window blinds or solid curtains in bedroom bathroom windows
  • Bedroom/bathroom door latches
  • Hallway clear and exits unobstructed
  • Water test report private well (6 yrs)
  • Any household members smoke?
  • Handicap accessibility- Ramps and bathrooms
  • Check if special diet need doctors order and guardian approval

    15. Collect 5 day visit form completed by CM and Nurse for program move or new program

     16. Certification:

  • Completed book review for regular and biannual certification
  • Schedule certification dates
  • Inform Managers and nurse certification date
  • Attend certification and support in certification process
  • Complete certification packet for regular and biannual certification
  • Collect nursing summery
  • Inform HR list of staff for certification
  • Collect background and insurance information from HR
  • Collect staff and provider current and previous car insurance
  • Collect staff and provider training and annual review

Required Skills:

  1. Good interpersonal skills able to communicate with all levels of employees.
  2. Must be able to utilize computer technology
  3. Must be detailed oriented and able to complete accurate work products on deadline
  4. Ability to independently manage multiple work tasks
  5. Strong Organization skills
  6. Must be able to maintain confidentiality
  7. Possess excellent written and verbal communication skills
  8. Ability to work a flexible schedule

Required Trainings:

  1. Social Role and Client Rights In-house training at hire and every three years
  2. Overview of DHHS at time of hire
  3. Defensive Driving at time of hire (within 6 months of hire date)
  4. New Hire orientation to include; Blood borne Pathogens, back Safety, Safety Program. HR policies and HIPPA at time of hire
  5. Department orientation at time of hire
  6. Role specific training to include, MOAB and other START trainings as determined by program needs
  7. Person Centered Thinking Training

 

Driving Level:

This position is a Level One (1) employee.  Level one employee:

  1. May occasionally  drive individuals in their car
  2. Only use their car on regular basis going back and forth between appointments
  3. Are required to take Defensive Driving at time of hire and every three years
  4. Are required to show proof of $100,000/$300,000 liability car insurance at time of hire and upon each expiration of the insurance date.

Physical Effort:

Must be able to sit or stand at a workstation.  Occasional walking may be required. 

 

Emotional Effort:

Must be able to maintain a positive and professional demeanor in a busy program environment.
